So what exactly is ‘Social Prescribing?'
Social prescribing is a way of linking you to non-clinical support in the community. Improving your lifestyle and making changes can be very challenging. You may not be sure where to start. Social prescribing can help to give you the confidence and support you might need to make a change.
There are a fantastic range of activities and organisations in the local community. Social prescribing is about linking you to these opportunities, with the aim of improving your health and wellbeing.
The Help at Hand App is up to date with the current support available. You can contact our social prescribing team via email at social.prescribing1@nhs.net
