We are pleased to inform you that you are eligible for the NHS Type 2 Diabetes Path to Remission Programme provided by Counterweight. This free 12-month programme will provide you with all the tools you need to help you lose weight, control your blood sugar, reduce your medication and potentially put your diabetes in remission. By joining the programme, you can prevent the development of any complications resulting from type 2 diabetes in the future.
You will receive one-to-one support from a dietitian or qualified nutritionist, and you can choose for this to be delivered in person or remotely (via video call or phone call). You will also be provided with a variety of resources via the Counterweight app, or physical workbook, where you can track your progress, set goals, access exercise programmes, read educational content, try new recipes and more!
The programme begins with 12 weeks of total diet replacement where all food is replaced with nutritionally complete meal replacements (soups, shakes and porridge) followed by a staged reintroduction to food, taking your lifestyle and preferences into consideration. Once you have completed the food reintroduction you will move on to weight loss maintenance for the remainder of the 12-month programme. Each phase will support you in building sustainable long-term changes alongside guidance from your coach.
The programme is funded by the NHS, including all meal replacements, so it is completely free of charge. However, as places are limited, we are encouraging everyone to apply as soon as possible.
