The aim of the project is to support residents by placing Community Health & Wellbeing Workers in specific areas of St Austell. If you are not on one of our roads then please refer yourself to the Social Prescribing Team who will be happy to help. We understand that life can be challenging at times. With support from a Community Health and Wellbeing Worker you will increase your support network, gain a listening ear and explore possible solutions to issues you may be facing.
