How to register with the practice
If you live within our practice area (please see the map below) you are welcome to register with us.
We are using a new online service called "Register with a GP Surgery".
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
The online service is only for permanent registrations. If you are wanting to register as a temporary resident, please see the information and form further down on this page.

Paper forms are still available if you need one, and are available at any of our sites.
You will receive a text message to confirm that your registration has been processed. If you need medical help, please let the receptionist know when you register.
St Austell Catchment area
Mevagissey Surgery Catchment area
You will have a named/usual GP who is responsible for your overall care. You will also have a preferred site (Foxhole, Mevagissey, Park or Wheal Northey). If you would like to know who your usual GP is, please ask reception. If at any time you wish to change your usual GP or preferred site, please let us know. Having a usual GP does not restrict your ability to see any GP in the practice.
Medical treatment is available from the date of registration. Your care will move to St Austell Healthcare at the time of completing your registration form.
When patients register with our practice we offer a new patient health check with one of our Healthcare Assistants who will check your weight, height, blood pressure, medical history and carry out any blood tests as necessary. If you would like a “New Patient Check” please arrange this with one of our receptionists.
We recommend that you download the NHS App. You can order repeat prescriptions and view test results online. You can nominate a pharmacy and we will send your prescriptions electronically to the pharmacy.

Out of Area Patients
If you live outside our practice boundary, we're open to considering your application. However, as an Out of Area patient, we won't have to provide home visits for you. We assess each application for this type of registration carefully, to make sure it is clinically appropriate . Typically, Out of Area patients are those who live elsewhere but work in St Austell.
It's important to understand that, as an Out of Area patient, you may need to use services available in your local area. If such a situation arises, you'll need to contact NHS 111. They will guide you to a local service, which could be a GP practice near where you live, an urgent care center, A&E, or a minor injuries unit.
If you live outside our practice boundary, we're open to considering your application. However, as an Out of Area patient, we won't have to provide home visits for you. Temporary Residents
If you are not registered with a doctor but need to see one, you can receive emergency treatment from any GP surgery. If you need treatment for more than 14 days, you will need to register as a temporary or permanent patient.
You can register as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient please contact our surgery on 01726 75555. There is a form to complete, which is available below. Practices do not have to accept you as a temporary patient. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
